Full product description
🏥 A practical, editable package covering 14 elements of GDPR documentation and procedures for a healthcare provider operating under Polish law. The materials must be completed and adapted to the provider’s actual practices before implementation. The package covers the protection of patient, employee and contractor data, the release of patient records and cooperation with external organisations. Who is it for?
  • For healthcare providers that are creating or organising their GDPR documentation and want to adapt a complete set of editable templates to their own processes.
  • If the organisation has several locations, unusual data flows, extensive telemedicine services or has experienced a data incident, an individual legal review is recommended before implementation.
What does our package include?
  • GDPR audit checklist;
  • Data Protection Impact Assessment (DPIA) for a healthcare provider;
  • Informacja o przetwarzaniu danych osobowych dla pacjentów;
  • Personal data security policy;
  • Personal Data Protection Rules;
  • IT system management instructions;
  • Data processing agreement;
  • Authorisation to process personal data;
  • Personal data breach register;
  • Record of processing activities;
  • Record of categories of processing activities;
  • Register of data-processing authorisations;
  • Register of data processing agreements;
  • Notification of a personal data breach to the data subject
+ BONUS — The package also includes a 30-minute consultation with Polish attorney-at-law (radca prawny) Anika Szczepańska on matters covered by the package. During the consultation, you can discuss how to complete and implement the documents covered by the package and ask questions about the GDPR and the applicable Polish data-protection requirements. Editable Word and Excel files — prepared for completion and adaptation before implementation. Why purchase the GDPR Package?
  • Better-prepared documentation — organised procedures and clauses help reduce the risk of gaps and breaches.
  • Better preparation for a data-protection incident — the documents help identify the steps to take and how to prepare a breach notification.
  • One consistent set — instead of individual templates and copies from different sources, you receive the 14 elements covered by the package.
  • Processes organised with GDPR in mind — the DPIA, registers, data-processing agreements and rules for granting access include elements to be adapted to the provider’s actual practices.
  • Support for onboarding — organised templates make it easier to induct new employees and contractors.
PRICE – PLN 599 including VAT

Important before implementation: the documents must be completed and adapted to the provider’s actual organisational and operating model. The package is neither a full GDPR compliance audit nor an analysis of the provider’s actual practices. An individual review under Polish law may be required for atypical or complex structures or following a data-protection incident.

Frequently asked questions

How and when will I receive the documents and invoice?

We send the ready-made files manually by email within 24 hours after payment has been credited. Sundays and statutory public holidays in Poland do not count towards this period. Saturdays count unless they are statutory public holidays in Poland. The files are not available as an automatic download immediately after payment. Fakturownia sends the invoice automatically, separately from document delivery.

How do I complete the documents?

The templates contain marked fields to complete and guidance. Enter the relevant details in the indicated places, using the explanations in each document. You can use the consultation included in the package to discuss selected sections and questions, within the time specified in the offer.

Can I get help completing the documents or arrange for them to be checked once completed?

Yes. This product includes 30 minutes of consultation. You can use this time to discuss selected sections of a document that you have already completed. The consultation does not include filling in or checking the entire set of documents. We agree additional help with completing documents or a broader review individually, confirming the scope and price before work begins. You can enquire even after buying one unit. For a larger purchase, you can ask about individual terms of engagement.

How many entities does a purchase cover, and what does buying multiple units provide?

One purchased unit of a document or package covers one entity / NIP (Polish tax identification number). You can use two units for two different entities. Consultation time adds up when you buy multiple units. Two units of this product include a total of 60 minutes; you can divide the time between entities.

Are the documents up to date?

We keep the documents up to date. Before sending them, we also check that the version being supplied is current. You receive templates that reflect the legal position verified before they are delivered.

When do the 12 months of free updates start, and how will I receive them?

For documents purchased in the shop, the free update period starts at the time of purchase. Once a new version is approved, the law firm sends the update to eligible buyers without them having to request it. For bespoke orders outside the shop, the scope and duration of updates are governed by the contract. This does not limit the buyer's statutory rights.

Can I reschedule my consultation?

Yes. You can reschedule an agreed consultation once free of charge if you notify the law firm at least 24 hours before it starts. Shorter-notice requests are considered individually. They do not automatically mean that your payment is forfeited. These rules do not limit the client's statutory rights.
